This new research project funded by
the National Council of Aging and the ACL Innovation Lab
studies if the negative impact of falls can be reduced by supporting the physical, cognitive and emotional health of older adults 60+ living in or around Rio Arriba County.

Ready to RISE:
follow the steps to enroll
Step 1. Fill out enrollment form
Step 2. Fill out and turn in the following forms

In person: come in and pick up the forms

Online: print out the forms and email them to us
Step 3. Come to an Orientation

In person & via zoom at Bridge to Health NM
-
Tuesdays at 5:00pm & Wednesdays at 11:00am
Step 4. Finish your Physical Assessment
Step 5. Start Activity Menu #1
If you ever feel dizziness or pain while doing any of the RISE exercises at home stop the activity immediately and call us right away

Why It Matters
Rio Arriba County has one of the highest fall-related hospitalization rates in New Mexico. Falls can impact your independence, but prevention, resilience, and safe recovery skills make all the difference.
